The consent form is valid for the entire school year. If your child was seen in the fall and you would like them to be seen again, YOU DO NOT NEED TO DO ANYTHING.
If your child was previously seen this past fall and do NOT want your child to receive dental services this spring, you will need to contact them by email at firstname.lastname@example.org or by phone at 505-553-3607. You will need to let them know before the planned arrival date.
For those that did not use these services in the fall but would like to have their child seen, please contact Stapleton Health Office and we will send a consent home with your child.

Dear Stapleton Families,
Several of you have asked about having non-Stapleton aged students at school. We appreciate those of you who take the time to volunteer during the school day. We need to ask that the little ones are not in the classroom since there is the potential to interrupt instruction.
If there are performances during the school day, you can certainly bring your small children. An example may be the upcoming talent show or a class performance.
You are more than welcome to bring your little ones to evening events and performances.
